ICS-214 Activity Log — Essential Documentation for Events & Deployments
The ICS-214 Activity Log is one of the most important forms you'll use during an event, deployment, or exercise. It's your personal record of what you did, when you did it, and who you worked with throughout the operational period.
For ARPSC events like the upcoming Simulated Emergency Test (SET) and other activations, we ask that all members complete and submit an ICS-214 form by the end of the event day via email and/or Winlink.
Why the ICS-214 Matters

- Legal Protection: Provides a contemporaneous record of your actions and decisions.

Carry a small, pocket-sized notebook with you during events and deployments. Use it to jot down notes throughout the day—you'll transfer these notes to your ICS-214 later.

- Time you checked in
- Time you took breaks
- Lunch and meal times
- Time you checked out
- Radio traffic you passed
- People you worked with
- Major observations or events
- Equipment used or issues
Understanding the ICS-214 Form Fields
